A Clinical Evaluation of the Pediatric Recurrent Intussusception

Abstract
Patients of intussusception were subdivided into those with recurrence(95 cases, 38 patients) and no-recurrence(239 cases, 239 patients) and the range of their age was from birth to 14 years old. And they were managed in the Departments of
General
Surgery and Pediatrics at hangang and Dongsan Sacred heart hospital from may 1991 to April 1994 and the results were as follows.
1) Overall recurrence rate was 13.7
2) Both the recurrence group and no-recurrence group have the peak incidence in less than one year old: the former is 81.6%, the latter is 74.1%.
3) The male incidence was predominant in both groups. In recurrence group, male was extremely predominant as the ratio 5.3 : 1.
4) Regarding to the previous associated diseases, clinical manifestations, and simple abdominal X-ray findings, there were no differences between two groups.
5) Ileo-colic intussusception was the most common type in two groups 92.5% of no-recurrence group and 96.8% of recurrence group.
6) 79.1% of no-recurrence group and 96.8% of recurrence group were managed within 24 hours from the onset of symptoms.
7) The clinical symptoms and signs were more prominent in no-recurrence group than in recurrence group.
8) In frequency, one recurrence was the most prevalent.
9) The recurrence of intussusception occurred within 6 months, 63.8%, within 1 year, 89.6%. Most recurrences were developed within 1 year.
10) The recurrence rate of barium reduction was 20.3% that of gastrografin reduction 25.0%, and that of operative manual reduction 4.3%. After surgical resection was made. Recurrence did not occur. The recurrence rate of operative manual
reduction
is
lower than that of barium or gastrografin reduction(P<0.005).
11) Nonoperative reduction (barium or gastrografin reduction) was successful in 74.1% of norecurrence group and in 89.5% of recurrence group. The success rate of nonoperative reduction is higher in recurrence group than in no-recurrence
group(P<0.01).
